James O´Neill stares at Alacran, but it is difficult for him to follow the fast talking Mexican´s words. And there seems to be two bloody Mexicans shouting at him at the same time!

-I ain´t talking to you, Mex, he says raising his left hand after placing the now empty glass on the bar. -Job? If you are looking for a job, Indian, dunno, you might get lucky with a land owner who needs an extra gun hand for protection. If you reckon you are decent gunslinger, ya know?

O´Neill opens and closes his right fist as he mentions the trade. There is no doubt he considers himself a better than average gunslinger.

-And since many around here have suffered Indian attacks, maybe they´d be keen on gettin´ one on their side for the change, HAHAHA! Listen, there´s a small town in the crossroads a few miles south. Local land owners post their to ask for gunslingers, or send their foremen there.

Then, he steps wide of the bar and adjusts his gun belt and belches loudly. The barman had replied to his query before with a nod, so he added the words that could make hell break loose in the Cantina: -Time for me to see the beautiful Rosanna, amigos!! I will warm her up for you!!, grabbing his crotch with his left hand.

(or a 10% penalty for skill checks with Wisdom or Dexterity as a relevant ability)

Reaction => low score he heads to the stairs, high he faces Alacran and Choctaw.

Things were definitely heating up inside the Cantina...

O´Neill was about to turn around and go upstairs, but some of the people inside had been listening to the exchange of words. And some knew Jose "Alacran" and his fixation with the girl Rosanna. The two Mexican next to him saw clearly how he was preparing to take out his gun, and they were holding their breath. The journalist was also listening to every word, and even the three cowboys stopped whatever they were doing when Alacran started swearing. All of a sudden, an eerie silence fell over the Cantina, interrupted only by the occasional scream of pleasure from upstairs.

O´Neil words impacted Alacran as if it were shots. Panic grips Alacran and looks in his eyes. He's in shock, but when he sees O´Neil turned toward the stairs, his expression changed to pure rage. The devil can be seen in his eyes. Then Alacran whispers. 

- Enjoy Rosanna...

Alacran starts to draw out a sawed-off Remington5, and Choctaw, the barman and the two Mexicans are the first to react. When Alacran has taken his gun out, and starts to aim at O´Neill15, the two Mexicans panic and they start to get up to run for the door.

More or less at the same time, Choctaw can see how the barman walks a few feet to a drawer to get a gun out20.

The cowboys on the other side do not react until Alacran has extended his arm with the shot gun, and the journalist is in a state of shock.

O´Neill, through his drunken haze, realizes something is terribly wrong when his eyes meet those of the journalist. But it is too late.

Count 15: Alacran starts to aim at O´Neill

Barman: he starts to act on Count 20 and he moves towards the drawer.
Mexicans: they start to act on Count 19 and 25

Choctaw: your action starts on Count 13, you cock the revolver in the holster (1 Count). You can see how the barman goes looking for a gun after count 20.

Alacran: your shot will be fired around Count 27. After Choctaw has declared actions, you can roll 1d20 and add +4 distance (about 15 feet), +4 for firing deliberately and +3 for your Accuracy, total bonus +11.

Choctaw takes his gun only for instinct, he aims to Alacran but doesn't fire and he gets a covered position near the barrels, and still aims the mex.

Alacran has his shot gun out, and FIRES!!! He is just off the mark to the right, but still hits O´Neill in the back, emptying both of the barrels with terrifying rage. Nearly a dozen pellets hit O´Neill´s right shoulder and back, and one of them comes out the other side with a pulp of flesh and veins. The gunslinger looses consciousness and fells deadly wounded on the journalist´s table. The young man from the East is all covered in blood now, and is petrified by the sight of a dead man on his table.

The two Mexicans had already stood up before Alacran fired, and now they are running towards the exit. They are about 10 feet away from their table34. Choctaw has his weapon out and he is ducking for cover close to the barrels in the corner. The Cantinero´s hand is shaking a bit, but he has taken a gun out from the drawer36.

The table with the three cowboys goes down heavily to the ground as the three men stand up. One of the stray pellets had grazed one of them, and they act instinctively getting up and going for their guns.

Choctaw: you have ducked for cover and ready for action on Count 34. From your perspective, you can see the barman has his eyes fixed on Alacran and probably will point at him. You can also see the the three cowboys going for their guns reacting to the shot gun blast.

Jack was giving it his all with the young Mexican girl. He had paid for a full hour and he was not going to waste any time. -Oh !! Yes!! Yess!!, she screamed as she grabbed the headboard.

Then suddenly BOOOMM!!!!!!!!

What the hell?!

A shotgun blast from downstairs!! Now all the upstairs girls were screaming and panicking, and Jack was putting his trousers back on.

Jack gets up from the bed, grabs his Winchester and runs to the door still naked. He slowly starts to open the door...

You start on Count 27 when the shot gun is fired. Each action takes "X" Counts of time (each count is 1/10 of a second). Initially, you rolled Initiative 7, so your first action starts on count 34. To get up from the bed is 3 counts, then take the Winchester (5+1d4=9), cock (1) and run to the door (6) and finally open the door.

Total 19 counts, so you will see what is happening downstairs on Count 53. I will describe the scene to you when you open the door (first everything need to be resolved downstairs, the shot gun was fired on Count 34 and different things can happen between count 27 and count 53, while you grab your gun and move towards the door).

EDIT: I just realized I put you as receiver of the post with the shotgun blast by mistake. Please ignore that part, I´ve fixed it now.
